2009 Notes
Over the past 6 months, hospital physicians (Drs. Wiley Robinson, Nasir Haque, Tapan Thakur, John Fowler and others) have discussed ways on how to improve the transfer of patients from hospital to nursing home and vice versa.

An informal look at the data from various hospitals found that 10-15% of LOS is due to delay in nursing home transfer.

In order to understand the complexity of the situation and help find workable solutions, Memphis Quality Initiative is working to put together a meeting of the key stakeholders.
We are reaching out to the Directors of Nursing at the local nursing homes and you at the hospitals.
